CHECKLIST:
• Provide dispatcher with an estimated repair time
• Confirm chassis number
• Confirm container number
• D.O.T. Information of on & off tire
• Cause of failure on all tire replacements
• Provide driver with work order stating services rendered (Do not include cost)
• Driver is to take replaced tire/parts unless otherwise specified
• Provide dispatcher with roll time when service is complete
